Why I Couldn't See For 2 Years: Keratoconus | What is Keratoconus?

In this video, board certified eye surgeon Dr. Michael Manning discusses with Dr. Yogendra what keratoconus is and ways of diagnosing and treating keratoconus. Keratoconus is an eye disease that affects the structure of the cornea, resulting in loss of vision. A cone-shaped cornea causes blurred vision and may cause sensitivity to light and glare. Keratoconus usually affects both eyes, though it often affects one eye more than the other.
Keratoconus occurs in approximately one in 2,000 individuals, typically beginning in puberty and progressing into the mid-30s. No one knows what causes keratoconus, although genetic and environmental factors are thought to be involved. Early stages can be treated with glasses, but with progression of the disease into late childhood and early adulthood, corneal transplantation may be needed to restore sight. Corneal collagen cross-linking is a procedure designed to stop the progression of keratoconus or slow it down.

Our recent research on long COVID (PASC) has identified persistent S1 spike proteins from SARS-CoV2 proteins in an immune cell called monocytes.

These monocytes act like “garbage trucks,” picking up anything foreign to the body, including viral particles.

We know that these monocytes travel all over the body, including crossing the blood-brain barrier and inflaming endothelial cells which line your blood vessels.

As the monocytes with COVID proteins migrate and irritate these blood vessels, they release pro-inflammatory cytokines and chemokines to signal more inflammation. One of these chemokines is called CCL5 or RANTES and is secreted by both the immune and endothelial cells. Another is called FRACTALKINE, which acts like velcro, causing the monocytes to stick to endothelial cells.

This process is similar to bees and honey, where RANTES acts as honey, and these monocytes with COVID proteins are like bees drawn to the honey. The IncellKINE test that we have developed picks up these immune signals and telegraphs us to the underlying cause of many of your symptoms.

Are you there for your Friend - Today was a good day | Mental Health COVID

There are tons of stories now of the deterioration of people’s mental health during the COVID pandemic. The lockdowns and lack of human interaction and contact have people, including children committing or contemplating suicide, and many more suffer from depression and anxiety. In talking to some COVID long haulers, many have also expressed hopelessness and depression, and there is so much uncertainty.

Throughout the years, when I felt down- breaking up with a girl, not getting a job, or into school I interviewed for, I would always remind ourselves to play Ice Cubes's Today Was a Good Day to put life into perspective and to keep on trucking along.

The only thing I can say is if you know someone who suffers from depression or suicidal thoughts, make sure they get the proper professional help and know you are there for them. It is a battle because they will recoil and push you away. But don’t give up on them, and don’t give up on each other.

And if you are feeling down or depressed, these lockdowns and the scaremongering from the media and social media getting to you, just look for anything small to hold onto- Whether it is going for a walk, eating your favorite cookie, dreaming about traveling, or just calling a friend- do anything that will give you and others hope.

I am constantly reminded that Matt was all about living in the present and making the best out of every day. And no matter what happens- good or bad-, as the great artist Ice Cube once said, “today was a good day.”

COVID Vaccines: 2 New COVID Vaccines You Should Know | Novavax and Johnson and Johnson Vaccine

By now, most of you have heard of the Moderna and Pfizer’s mRNA COVID vaccines, which were the first 2 COVID vaccines to get emergency use authorizations or EUA from the USA FDA. Obviously, both companies set a high bar, achieving around 95% efficacy in their covid vaccine trials, making it a huge challenge for all vaccine developers.

But many other covid vaccines are being developed. Some like Novavax & Johnson and Johnson vaccine conclude their phase 3 clinical trials within the next few weeks. If the results are encouraging, they should also be applying for approvals.

The data from Johnson and Johnson vaccine Phase 3 covid vaccine trial, called Ensemble, is perhaps the most-anticipated pharmaceutical study data of the early part of 2021. According to JJ, they believe they can inoculate 1 billion people against the virus.

The covid vaccine uses an altered version of a cold virus called Adenovirus26 to carry DNA for a piece of the SARS-CoV-2 virus into the patient’s cell, a different platform than Moderna Pfizer, w use an mRNA genetic code encapsulated in lipid nanoparticles.

The other covid vaccines to be on the lookout for is NVX-CoV2373, made by Novavax. NVX-CoV2373 is a two-dose protein subunit vaccine with the second shot administered after 21 days. They are currently in a phase 2b trial in South Africa and two phase 3 clinical trials- 1. In the UK and the second in the US and Mexico. The South African-based phase 2b trial results are expected to be released by the end of January 2021, and the UK phase 3 interim data is expected in early 2021.

NVX-Cov2373 contains a full-length, prefusion spike protein made using Novavax’s recombinant nanoparticle technology and the company’s proprietary saponin-based Matrix-M adjuvant. The purified protein is encoded by the SARS-CoV-2 spike protein's genetic sequence and is produced in insect cells. It can neither cause COVID 19 nor can it replicate, is stable at 2°C to 8°C, and is shipped in a ready-to-use liquid formulation that permits distribution using standard vaccine supply chain channels.

So these are the second wave of vaccines that I expect to next get FDA EUA’s. Now keep in mind, there are many other covid vaccines in the pipeline, such as Inovio, Generex, and Tonix Pharmaceuticals, that all use different platforms. If their clinical trials are promising, we should see approval too. I have had some interviews on my show with the management of Generex and Tonix pharmaceuticals, so I will post links below to learn a little about their promising technology.

The topic in this video is the Chronic COVID (Long Haulers) Center program that Dr. Bruce Patterson, the CEO of IncellDX, myself, and a few of our partners are launching by the middle of November 2020.

Several months ago, many of you on social media brought to my attention COVID survivors who were now complaining of a new onset of symptoms ranging from fatigue and malaise to brain fog, burning sensation under their skin, hair loss, cardiac and respiratory problems (LONG COVID). It is the long-term effects of covid on the body.

The startling thing for me was how these symptoms persisted in people weeks and months after infection. Many shared with me how their health providers ran them through a gauntlet of tests if they were even offered tests in the first place, and when these were negative, they were dismissed as anxiety or hypochondria or stress.

I started investigating to see if there were chronic complications from previous coronavirus outbreaks, specifically SARS and MERS. While there were limited studies because these epidemics were quickly contained, there were suggestions that the etiology of some of these complications was due to pro-inflammatory cytokines.

And what we discovered a heterogeneous pattern of elevated cytokines compared to normal controls in chronic COVID patients weeks and months after their initial infection.

Bucillamine (N-(mercapto-2-methylpropionyl)-l-cysteine), which has a well-known safety profile and is prescribed in the treatment of rheumatoid arthritis in Japan and South Korea for over 30 years, is a cysteine derivative with 2 thiol groups that is 16-fold more potent than NAC as a thiol donor in vivo, giving it vastly superior function in restoring glutathione and therefore greater potential to prevent acute lung injury during influenza infection. Bucillamine has also been shown to prevent oxidative and reperfusion injury in heart and liver tissues and is highly cell permeable for efficient delivery into cells. Bucillamine has unrealized potential for the treatment of influenza with both proven safety and proven mechanism of action similar to that of NAC, but with much higher potency, mitigating the previous obstacles to using thiols therapeutically. It is also reasonable to hypothesize that similar processes related to ROS are involved in acute lung injury during COVID infection, possibly justifying the investigation of bucillamine as an intervention for COVID-19.

The FDA Phase 3, multi-center, randomized, double blind, placebo controlled, clinical study of bucillamine (2 dosage levels) in patients with mild-moderate COVID. Patients will be randomized 1:1:1 to receive bucillamine 100 mg 3 times a day (TID), bucillamine 200 mg TID or placebo TID for up to 14 days. After the first interim analysis when a single dose is selected, patients will then be randomized 2:1 to the selected bucillamine dose or placebo The study will be overseen by an independent Data and Safety Monitoring Board (DSMB). Up to 10 centers in the United States will conduct this study. Up to 1000 patients will be enrolled in this study. Patients will participate in the study approximately 45 days.

8 Key Criteria to Help You Assess New COVID Medications | Covid Medication Update

I share the 8 criteria I look for in understanding new experimental COVID drugs that help me understand if they can treat, not just covid. Still, any disease it is being developed for. This is not meant to be a pharmacology lesson but just a guide to help you assess information.

Many other new experimental therapeutic COVID drugs and COVID vaccines are in the pipeline from many different companies. It can also be very confusing and overwhelming to understand what is really going on.

I look at the mechanism of action, a pharmacological term, to describe how the drug works on a molecular level.

The second criteria are pharmacovigilance, which is commonly known as drug safety.

The third criterion is “does the drug work,” and there are two pharmacology concepts that I use as a guide: efficacy and effectiveness.

The fourth criterion is administration, which is how the drug is delivered into the body.

The fifth criterion is wherein the developmental phase is the drug.

The sixth criteria are inclusion and exclusion criteria, a set of predefined definitions used to identify patients who will or will not be included in the study.

The 7th and 8th criteria are price and scalability.

Heart Rate Variability Training with Dr. Marco Altini | How to Exercise Using HRV

Heart rate variability is a noninvasive electrocardiographic method that can be used to measure your body’s adaption to stress and exercise. HRV is simply a measure of the variation in time between each heartbeat. This variation is controlled by a primitive part of the nervous system called the autonomic nervous system (ANS).

HRV can identify these variations and imbalances. For example, if a person’s system is in a fight-or-flight mode, the variation between subsequent heartbeats is low. On the other hand, if one is relaxed, the variation between beats is high. In other words, the healthier the ANS, the faster you can switch gears, showing more resilience and flexibility. Over the past few decades, research has shown a relationship between low HRV and worsening depression or anxiety. A low HRV is even associated with an increased risk of death and cardiovascular disease.

Research has shown a strong correlation between low HRV values and increases the risk for diabetes, coronary disease, depression, and anxiety. Low HRV scores are also associated with impaired regulatory and homeostatic ANS functions that reduce the body’s ability to cope with internal and external stressors. Higher HRV scores correlate with increased human peak performance, fitness levels, and improved rest and recovery.

Recovery is more complicated than an isolated measurement, and looking at daily HRV values does not indicate your level of recovery or how well you handle stress. In addition, it is difficult to determine how you respond to stress and adaptation just by looking at individual HRV values. So it is important to first establish baseline HRV values over a minimum of one week to understand how your body’s response to stress and other stimuli.

The gold standard is to analyze a long strip of an electrocardiogram, the test we frequently do in the medical office where we attach wires to the chest. But Dr. Marco Altini has developed a clinically validated app called HRV4Training in the Apple and Android app store, where you can use your phone/iPad camera to measure your heart rate variability.

There was big news out of India when the Drug Controller General of India (DCGI), approved the drug Itolizumab for emergency use for the treatment of cytokine release syndrome or the cytokine storms in patients with moderate to severe ARDS due to COVID-19.

Itolizumab is an anti CD6 monoclonal antibody that was approved by the DCGI for the treatment of moderate-to-severe chronic plaque psoriasis in 2013.

And according to Biocon’s chief medical officer, Dr. Sandeep Athalye,

itolizumab demonstrated a statistically significant advantage over the control arm, in one-month mortality rate. Key efficacy parameters such as PaO2 and SpO2 (oxygen saturation) improvement without increasing FiO2 (oxygen flow) also showed a statistically significant advantage for Itolizumab arm over the control arm. All the patients on Itolizumab arm were weaned off oxygen by day-30, and none needed ventilator support, unlike the control arm. Key secondary endpoints of clinical markers of inflammation such as IL-6, TNF-α, serum ferritin, d-dimer, LDH and CRP showed clinically significant suppression post-dose and correlated well with clinical improvement in symptoms and chest x-ray images. Itolizumab was overall well tolerated and was found to be safe. Itolizumab when administered to patients with moderate to severe ARDS due to Covid-19, prevents morbidity and mortality due to cytokine storm.

COVID 19 Therapeutic Treatment: The Potential of Lenzilumab | Cytokine Storms:
Discussion about a stimulating new drug called Lenzilumab by the biotech company, Humanigen which is currently in FDA phase 3 covid 19 trials. Lenzilumab is a monoclonal antibody against granulocyte-macrophage colony-stimulating factor or GM-CSF. In a 12 covid 19 patient study at the Mayo Clinic, treatment with Lenzilumab showed promise in severe and critical COVID 19 pneumonia patients.

It is important to understand this progression because while the symptoms of the early phase are due to the covid 19, the later phases are immunologically driven by cytokine storms. As I have discussed in previous videos, cytokines are messenger proteins made by the immune cells to coordinate immune responses. For an unknown reason, some people are more susceptible to these cytokine storms, which can be described as a hyperimmune response to the covid 19. These cytokine storms have been implicated in acute respiratory distress syndrome, cardiac, liver, and kidney damage, along with strokes and blood clots.

So that is why in the early phase of the COVID 19, antivirals like hydroxychloroquine and Remdesivir have shown some promise. In the later phases, they haven’t because it is the cytokines and the cytokine storms causing the pathology and mortality. This is why there has been a focus on therapeutics to quell these storms and regulate the immune system.

The cytokine pathways are very complex, and identifying the main cytokines involved is the biggest challenge doctors, and scientists face. As I have discussed in previous videos about Dr. Bruce Patterson’s research identifying the chemokine, RANTES, as the main cytokine involved in these storms, the researchers at Humanigen have identified another key cytokine responsible for the immunological damage in COVID 19: granulocyte-macrophage colony-stimulating factor or GM-CSF.

GM-CSF is a myelopoietic growth factor and pro-inflammatory cytokine. It plays a critical role in alveolar macrophage homeostasis, lung inflammation, and immunological disease, and it is believed Lenzilumab blocks this cytokine, thus potentially quelling the cytokine storm.

GM-CSF is a cytokine and an important hematopoietic growth factor and immune modulator. It plays a key role in the differentiation and proliferation of monocytes, macrophages, granulocytes, and elevated GM-CSF levels are associated with autoimmune and inflammatory diseases and cancer.

GM-CSF has been described as

A communication conduit between tissue-invading lymphocytes and myeloid cells.
Even though lymphocytes are in all likelihood the instigators of chronic inflammatory disease, GM-CSF-activated phagocytes are well equipped to cause tissue damage.

Long Term Effects of Covid 19 on Health - Why are some people not recovering?
There are growing numbers of recovered COVID 19 patients called "long haulers" who suffer from infectious complications. This will be a problem we have to address as more and more people recover from their COVID 19 infections.

Cardiac, pulmonary, endocrine, coagulation, rheumatological, and in some cases, chronic fatigue syndrome is being reported, but very attention is being paid to these patients. See, when we get a cold or feel sick, after a few days, most of us are back to normal and our regular routines. Some COVID 19 patients have not gotten back to normal and are struggling to get on with their lives months and weeks after the infection.
While the etiology is unknown and patients are being told “these are just the effects of the virus,” but could elevated cytokines be the reason for these complications?

Researchers now believe that COVID 19 starts out as a viral disease then becomes an immunological disease caused by a hyperactive immune system led by cytokines. So the old paradigm is once you test negative for COVID 19, the risk for elevated cytokines and cytokine storms decreases. But I think this paradigm is incorrect and why the inflammatory-like conditions in post-COVID 19 patients are being ignored.

However, since doctors are either not testing or refusing to test recovered COVID 19 patients for their levels of cytokines, we do not know if cytokines are truly the reason for post-COVID 19 complications. But this is the reason we should be measuring cytokine levels and, more specifically, Interleukin-1, Interleukin-6, Interleukin-8, RANTES, and MCP-1, not only inactive and acute COVID 19 infections but also when a patient is discharged. As I have said many times, nonspecific inflammatory markers like serum ferritin and C-reactive protein can help a diagnostic marker. Still, they do not paint the complete picture.

But I feel that we will be seeing an increased number of post-infection complications over the next several months as more and more people recover from COVID 19. Dr. Patterson sees cytokines and other inflammatory markers in patients 30-40 days post-infection, and some lab reports I have personally seen show elevations even 90 days later. But we will not prove or disprove cytokine's role in post-infection complications unless doctors and hospitals start testing.

A new and exciting experimental antiviral and covid 19 drugs called Neumifil are being developed in Scotland by a company called Pneumagen could be the game-changing antiviral medication to treat and prevent COVID 19 and influenza infections.

Neumifil uses Pneumagen’s platform technology to develop glycan targeted carbohydrate-binding module domains to prevent and treat respiratory tract infections. Basically, Neumifil masks the glycan receptors in your airway to stop the covid 19 and influenza from entering the cells.

There are glycan receptors located on the surface of lung cells, and along with the ACE2 receptors, they are believed to be how covid 19 enters your cells. When someone gets exposed to COVID 19, the key initial step in the process is when the virus latches onto the glycan receptors on the cell.

Neumifil has been administered intranasally in preclinical models and has demonstrated prevention, treatment, and post-exposure prophylaxis of influenza and RSV infection in the lab. Pneumagen’s lead researchers have suggested that Neumifil would be administered as a nasal spray given weekly or every other day.

While clinical trials on humans are not expected to begin until 2021, Neumifil is a drug I would highly recommend keeping a close eye on because it can be prophylaxis and treatment for many respiratory tract infections, including covid 19 and influenza.

COVID 19 and Anesthesia | COVID 19 Management in The Operating Room and ICU | What is Anesthesia?
Because anesthesiologists are the critical care doctors of the operating room, during the COVID 19 pandemic, many of us transitioned to managing COVID 19 patients in the ICU while at the same time managing emergency surgeries for the same patients.
So in this video, I will go over what anesthesia is and what do anesthesiologists really do, and our role in the COVID 19 pandemic.

During COVID 19 pandemic, most elective surgeries were canceled, and anesthesiologists transitioned from working in the OR to the ICUs and emergency rooms. Remember, anesthesiologists are also critical care physicians in the operating room, and some, in fact, works predominantly in the ICU over the OR.

In some hospitals, anesthesiologists formed airway teams where they were the only providers responsible for intubating COVID 19 patients. Intubation is also a very high-risk procedure in COVID 19 because we are extremely close to someone’s mouth and airway.

There were still emergency surgeries taking place and sometimes involving COVID 19 patients. The challenges were not only to medically manage COVID 19 patients but also to maintain strict infection control.
The other challenges included ventilating patients with severe ARDS from COVID 19 infections to maintaining their blood pressure and heart rate despite being on life-supporting medications like epinephrine and Levophed.

To become an anesthesiologist in the United States, one must complete pre-medical courses in undergrad, which includes all your basic sciences and math. During your senior year of college, you take the MCAT exam and apply to medical school. Some will take a few years off to do research and graduate school like I did when I got my master's in public health. Medical school in the United States is a 4-year program broken down into 2 years of basic sciences and 2 years of clinical rotations. There are 4 exams you take to become eligible for a medical license.

After medical school, we enter a 4-year anesthesiology residency program. The first year of residency is also called “the intern year,” and we rotate through internal medicine, surgery, ICU, and the emergency room. The next 3 years are our clinical anesthesia training. We spend many months rotating through the operating room and ICU with intense training in cardiology, pulmonary, pediatrics, pain management, and OB.

After residency, we take 2 exams to become board certified by the American Board of Anesthesiology- a written exam and an oral exam.

Dr. Mike Hansen and I (Dr. Yo) had Dr. Bruce Patterson from INCellDX on our show talking about coronavirus (COVID 19) and, more specifically, how the chemokine RANTES was the key player in the pathogenesis.

Dr. Bruce Patterson’s research and manuscript from the Leronlimb study from Montefiore Hospital are actually providing us with a blueprint for the pathogenesis and an approach to treatment for COVID 19.

Since our live session was close to 90 minutes, I wanted to discuss 5 major points that are stood out for me.

The first major point was Dr. Patterson defining COVID 19 as a “RANTES driven disease.” RANTES is a type of cytokine called a chemokine and acts to attract T-cells and other immune cells to the areas of inflammation and infection.

So the second major point Dr. Patterson talked about was how our blood supply might be contaminated.

The major third point from his talk was the safety issue between Leronlimab versus some other medications used to treat COVID 19.

The fourth major point was how Leronlimab also acts as an immune modulator.

The fifth major point and might be the biggest one was knowing that there are not many studies yet looking at Leronlimab and COVID 19. We know that anecdotally there are some case reports from UCLA and Montefiore showing Leronlimab having success in moderate to severe COVID 19 patients.

Heart rate zone training programs, especially Zone 2 Training, focus on training your body to optimize your metabolism, build a strong aerobic base, and developing well-rounded physiology. And by doing this, your body can efficiently break down fat, workout harder, and keep that weight off.

Are you one of those people who go to the gym a couple times a week, eat a specialized diet, and takes supplements, yet you don’t see the results you want?
A big part of that reason is we are taught to exercise to only lose weight. This video talks about a heart rate training program you can use to help you lose weight and develop adaptations to your physiology and optimize your metabolism to continue to keep that weight off.

Zone 2 Training
Zone 2 training is 60-70% of your maximum heart rate. The purpose of Zone 2 training is to develop physiological adaptations and build an aerobic base to keep the weight off and optimize your metabolism.
You won’t see some of those dramatic weight loss results from only Zone 2 workouts, so that’s why I believe in starting out first with Zone 4 and 5 training to see those initial results and then incorporating Zone 2 workouts.

There are 2 main physiological adaptations with Zone 2 training:

1. You train your body to use fat more efficiently as an energy source. In low heart rate zones, the main substrate your body uses as energy is slow-burning fats. And working out in the slower Zone 2 helps your body adapt to burning fats more efficiently. And when you are not at the gym or working out, your body has now adapted to better dealing with fats, and this can help you

2. You increase the mitochondrial density, which helps delay lactic acid formation. Mitochondria are structures in your cells where the energy molecule ATP is formed and where lactic acid is cleared. Physiological studies have shown that exercising in Zone 2 will increase the size of your mitochondria and causes them to clear lactic acid faster. And this will help you work out longer and harder without fatigue and thus make you lose more weight and calories.

Total calories burned is the key to weight loss, so to maximize your time, working out for the next 45 minutes in Zone 4 and 5 will increase the total calories burned in the shortest amount of time. Zone 4 is 80-90% of your maximum heart rate, and Zone 5 is 90-100%. I have made a much more detailed video on these Zones and how to calculate your maximum heart rates, so I will post a link to that video in the description section of this video.

What some may not know is a few weeks ago their friend got in touch with me seeing if I could help. COVID 19 cytokine storms increase mortality. If vitamin D plays a role in decreasing pro-inflammatory cytokines, this video explores how vitamin D deficiency increases the susceptibility for a COVID 19 patient developing cytokine storms?

Vitamin D is a fat-soluble vitamin that plays a role in calcium absorption, bone health, and immune function. Vitamin D comes in two forms: vitamin D3, which we get from sunlight and animal products, and vitamin D2 through our diet like milk, fish, and fortified cereals. Vitamin D supplements can come in either vitamin D2 or D3. While some experts have suggested that vitamin D3 is more potent and absorbed better, either type is ok if it takes general health.

Vitamin D2 mainly comes from fortified foods like breakfast cereals, milk, and other dairy items. Vitamin D3 is made by your own body when UVB light from the sun penetrates 7-dehydrocholesterol in the skin. Vitamin D3 is also found in some foods, including eggs and fatty fish, such as salmon, tuna, and mackerel.

Both Vitamin D2 and D3 are then bound to a carrier protein called vitamin D binding protein (DBP) and then metabolized in the liver into calcifediol, also called 25-hydroxyvitamin D. When your doctor orders lab tests to check your vitamin D levels, they are measuring your 25-hydroxyvitamin D levels.

Vitamin D status is significantly associated with clinical outcomes. The results suggest that an increase in serum 25 hydroxyvitamin D level in the body could either improve clinical outcomes or mitigate worst (severe to critical) outcomes. In contrast, a decrease in serum 25 hydroxyvitamin D level in the body could worsen the clinical outcomes of COVID 19 patients. In conclusion, this study provides substantial information to clinicians and health policy-makers. Vitamin D supplementation could possibly improve the clinical outcomes of patients infected with COVID 19.

Does sunlight kill COVID 19, and do the warmer summer months offer protection against it?

Many infectious diseases, especially influenza, peak during the winter months between November and March in the Northern Hemisphere and between May and September in the Southern Hemisphere. But studying these patterns cannot be viewed in a vacuum, and the challenge is identifying the factors that play a role: covid 19, the environment, and human behavior. For example, could these infections increase in the winter season because of less vitamin D from the decrease in sun exposure? Maybe it is the many holidays during these months bringing people closer together and increasing the chance to spread infection.

In a study from Edinburgh, United Kingdom, looking at other human covid 19, researchers suggested:

“covid 19 displayed marked winter seasonality between December and April and were not detected in summer months, which is comparable to the pattern seen with influenza viruses.”

William Bryan, science and technology adviser to the Department of Homeland Security, said that government scientists found ultraviolet rays had a potent impact on the pathogen, offering to hope its spread may ease over the summer.

It has long been known that ultraviolet light has a sterilizing effect because the radiation damages the virus's genetic material and its ability to replicate. But the bigger question is “what ultraviolet light” is most effective.

See, we know that sunlight has 3 ultraviolet wavelengths:

UVA and UVB penetrate the Earth’s surface and are the reason why you get sunburns.
UVC light has a short wavelength, so it doesn’t penetrate Earth’s surface and inactivates many deadly viruses.

UVC is used by some hospital disinfecting robots, but because it is hazardous to the skin and eyes, many experts advise caution.

We need to know if UVA and UVB light that reaches the Earth’s surface was the light that government scientists found. If that is the case, then the summer months might potentially see a decrease in COVID 19 infections. But if it is UVC light that doesn’t penetrate the Earth’s atmosphere, we will be disappointed.

Many health experts say a COVID 19 vaccine could be available in 12-18 months, while others have suggested they could be available as early as the summer of 2020.
So when can we expect a COVID 19 vaccine?

If you are talking about a mass covid 19 vaccination campaign, then the 12-18 month timeline is the most realistic. However, there are many new experimental COVID 19 vaccines currently in clinical trials that could be available to high-risk groups as early as June or July of 2020.

Some of these experimental vaccines were already being investigated for the previous covid 19 outbreaks: SARS and the MERS, so biotech companies are not starting from scratch and can quickly roll out clinical trials. Currently, there are 115 vaccine candidates, but only a few of them are in a clinical development phase, meaning many of them are still in the exploratory stage.

Covid 19 vaccine stimulates your immune system to produce COVID 19 antibodies, exactly like it would if exposed to COVID 19. After getting vaccinated, you develop immunity to COVID 19 without having to get the disease first.

Vaccines work by mimicking disease agents and stimulating the immune system to build up a defense against them.
Vaccination “programs” the immune system to remember a particular disease agent by allowing it to “practice” on a weakened or killed version of the pathogen and develop antibodies against it. This is called a primary response to a pathogen.
So if the pathogen invades the body again in full strength, the immune system is ready to respond with a swift and specific defense. This is called a secondary response to a pathogen. Secondary responses happen faster and at a greater magnitude than primary responses.

When the covid 19 vaccine antigen is injected into you, a group of cells called Antigen Presenting Cells or APC ingests the vaccine antigen, breaks it apart, and displays a piece of the antigen on its surface. APCs are usually macrophages, B-cells, and dendritic cells, and they are roaming the body looking for invaders.
The APCs activate T-Helper Cells, which activate naïve B-cells to transform to plasma B cells to produce antibodies specific to the vaccine antigen.
If the vaccine contained attenuated or weakened viruses, the activated T-helper cells also stimulate naïve Killer T Cells to transform to active killer T cells and destroy the weakened virus in the vaccine that had infected your cell. These functions lead to the creation of memory B-cell, memory t helper cell, and memory killer t cell. The goal of immunization is to produce a memory of the vaccine antigen via a large memory cell population.

A potential cause of COVID 19 fatalities is the cytokine storm which is an out-of-control immune system. What is happening in covid 19 and cytokine storm is your body’s own immune system is betraying you by going into overdrive and attacking COVID 19 and your own healthy cells.

Cytokines are cell-signaling molecules that aid cell-to-cell communication in immune responses and stimulate the movement of cells towards sites of inflammation, infection, and trauma caused by COVID 19.

The cytokine network involves several different cell types that coordinate their efforts as part of the immune system. While this complex network can look intimidating at first, the point here is that cytokines may act on the cells that secrete them, called autocrine action, on nearby cells called paracrine action, or in some instances on distant cells called endocrine action. Cytokines can be pro-inflammatory or anti-inflammatory and also be involved with nerve sensitization causing Pathologic pain.

Proinflammatory cytokines are produced primarily by activated macrophages and are involved in the up-regulation of inflammatory response.

INTERLEUKIN1β, INTERLEUKIN-6, and TUMOR NECROSIS FACTOR ALPHA are the typical proinflammatory cytokines.

The major anti-inflammatory cytokines include INTERLEUKIN-4, 10, 11, 13, and TRANSFORMING GROWTH FACTOR. IL-10 is a potent anti-inflammatory cytokine and suppresses proinflammatory cytokines such as TNF-α, IL-6, and IL-1.

Tocilizumab is a novel monoclonal antibody that competitively inhibits the binding of interleukin-6 (IL-6) to its receptor (IL-6R). Inhibiting the entire receptor complex prevents IL-6 signal transduction to inflammatory mediators that summon B and T cells.
Tocilizumab is already approved by the FDA to treat cytokine release syndrome (CRS) that is severe or life-threatening, and they recently approved a phase 3 clinical trial for COVID 19 pneumonia.

What personal protective equipment you use depends on the patient, their condition, and the type of activity you are engaging with them in healthcare.

All healthcare workers follow standard precautions when taking care of all patients.

. Standard Precautions are the minimum infection prevention practices that apply to all patient care, regardless of suspected or confirmed infection status of the patient, in any setting where health care is delivered.

A surgical mask is a loose-fitting, disposable device that creates a physical barrier between the mouth and nose of the wearer and potential contaminants in the immediate environment. These are often referred to as face masks, although not all face masks are regulated as surgical masks.
An N95 respirator is a respiratory protective device designed to achieve a very close facial fit and very efficient filtration of airborne particles. Note that the edges of the respirator are designed to form a seal around the nose and mouth. The 'N95' designation means the respirator blocks at least 95 percent of tiny (0.3 microns) particles. If properly fitted, the filtration capabilities of N95 respirators exceed those of face masks. However, even a properly fitted N95 respirator does not completely eliminate the risk of infection and illness.

People are dying of COVID because of multiorgan failure, pneumonia, and ARDS. We have been told that the lungs are central to the disease pathology. But what if there is something more sinister going on involving your red blood cells, more specifically, involving hemoglobin? We have to start by looking at how hydroxychloroquine works in malaria.

Hydroxychloroquine kills the malaria parasite by targeting hemoglobin. So if hydroxychloroquine is effective against COVID, could its mechanism of action also be on hemoglobin, just like in malaria, and if so, does that mean COVID's real underlying pathology involves hemoglobin?

Inside your red blood cell, there are about 200-300 million hemoglobin molecules. Hemoglobin is a protein made up of 2 components: heme and globin. The globins are proteins that fold into a structure called subunits. Each hemoglobin molecule has 2 alpha-globin subunits and 2 beta-globin subunits. Because there are 4 subunits, each hemoglobin molecule can reversibly bond with up to 4 molecules of oxygen.
The heme consists of an iron (Fe) ion surrounding a porphyrin ring made up of 4 nitrogen-containing pyrrole molecules.

Malaria is caused by a single-celled parasite, Plasmodium, that a mosquito carries.
When a mosquito bites you, a sporozoite, the Plasmodium parasite's immature form, enters your bloodstream. The sporozoite then travels to the liver, where it enters into liver cells called hepatocytes.
It is not fully understood how chloroquine kills these malaria parasites. Still, it is believed that chloroquine, which is a weak base, accumulates in the parasite's acidic food vacuole, thus killing it. In addition, when Plasmodium breaks down hemoglobin to heme and globin, the heme portion is toxic to the malaria parasite. To prevent itself from being damaged by heme, the parasite produces a chemical to convert heme to a non-toxic compound. Chloroquine blocks this chemical's action, which causes the toxic heme levels to rise, thus killing the malaria parasite.

When COVID Virus enters your cell, it takes a piece of your cell’s surface membrane, covers itself up, and is now referred to as an endosome vesicle. Imagine someone ripping off a piece of your skin and covering themselves up with it as a way to protect themselves. Endosomes become increasingly acidified as they move from the cell surface further into the cell’s interior. The fusion of coronaviruses within endosomes depends critically on the acidic environment.

Are your Amazon Packages and other packages you order on the Internet safe from COVID?
So what is the risk of contracting COVID through your food and packages?

Note - After all these foods and packages have been touched by multiple people- the people who put your food in the packaging or the Amazon assembly line worker processing your order, the person who loaded the truck, and the UPS or FEDEX worker who delivered your package.

So in this video, I will go over these risks and give you my advice and recommendations on how to handle supermarket foods and produce and your packages from the Internet.

So as I have discussed in other videos, it is believed that COVID is spread primarily through respiratory droplets and touching contaminated surfaces.

However, according to the CDC:
“Transmission of the new coronavirus to persons from surfaces contaminated with the virus has not been documented. Transmission of COVID occurs much more commonly through respiratory droplets than through fomites. Current evidence suggests that COVID may remain viable for hours to days on surfaces made from a variety of materials.”

The CDC also says that cleaning of visibly dirty surfaces followed by disinfection is the best practice measure for the prevention of COVID and other viral illnesses in the household and community.

Cleaning refers to removing germs, dirt, and impurities from the surfaces, but it does not kill germs.
Disinfecting refers to chemicals used to kill germs on surfaces but doesn’t necessarily clean dirty surfaces.

There is some evidence that suggests COVID can travel in the air and spread via airborne transmission. If COVID can spread via the airborne route, this poses an even greater challenge in preventing transmission of COVID.

We were initially told that coronavirus could only be spread via respiratory droplets and by touching contaminated surfaces. Respiratory droplets refer to your saliva and mucus and can spread as far as 6 feet away from you.
If you are infected with COVID, when you cough or sneeze or talk, the coronavirus will travel in your saliva and mucus. And others can get sick when they come in contact with those infected droplets but it doesn’t linger in the air that long because the weight of your saliva and mucus along with gravity will make it fall down quickly. And this is the reason for the 6-foot social distance health officials have been advocating for weeks and months

Respiratory infections can be transmitted through droplets of different sizes: when the droplet particles are greater than 5-10 micrometers in diameter they are referred to as respiratory droplets, and when they are under 5 micrometers in diameter, they are referred to as droplet nuclei. According to current evidence and guidelines, the current coronavirus is primarily transmitted between people through respiratory droplets and contact routes.
